- Location: Hybrid: Three (3) Days Remote/Work from Home & Two (2) Days in Manhattan, NYC (Midtown East 51st St.)
- Schedule: Monday – Friday / 8:30 am – 5:30 pm
- Salary: $55 - $75 per hour
Requirements:
- Current New York Registered Nurse (RN) License
- Associates or Bachelor's Degree in Nursing (ASN or BSN)
- Minimum of three (3) years as an Intake clinician or related experience in community health, health care administration, Long-Term Home Health Care Program or discharge planning
- Ability to balance multiple tasks and manage conflicting priorities
- Demonstrated strong interpersonal, customer service and relationship building skills
- Strong attention to detail and follow-up skills
- Basic computer/PC skills, as well as experience with EMR/EHR software systems
Job Description
- Provides guidance and support to intake staff in managing the day-to-day operations of assigned intake staff to ensure safe transition of patients into home care services.
- Assists in the coordination of home care services for complex cases, facilitates problem resolution
- Develops staff through individual and team coaching; involves staff in the process of self-development; facilitates regular staff meetings to hear staff updates and concerns; and offers guidance, input and suggestions on best practices.
- Ensures quality of intake staff performance is consistent with mission, policies and standards and in compliance with regulatory requirements.
- Identifies, develops and implements strategies and actions to improve intake staff performance.
- Manages, monitors and audits referral/intake processes to ensure timeliness, accuracy, procedural compliance and transmittal of referrals to appropriate program.
- Implements and manages documentation and reporting systems for all referral sources.
- Interprets health policies and procedures for intake staff and referral sources.
- Collaborates with Business Development senior management and/or referral sources to identify, develop and implement strategies for improving the efficiency and effectiveness of the intake program.
- Collaborates with account management to achieve admissions targets.
- Assists in the development of long- and short-range strategies and objectives for intake program.
- Implements strategic initiatives with Business Development and Operations teams
- Performs all duties inherent in a managerial role.
- Ensures effective staff training, evaluates staff performance, provides input for the development of the department budget, and hires, promotes, terminates staff and recommends salary actions, as appropriate.
- Participates in special projects and performs other duties as assigned.
