JOB SUMMARY
Responsible for the overall coordination of patient care while in residential services, including integration with family services, sessions, communication with referral sources, assessment of patient progress and communication to the clinical team regarding special treatment needs and procedures.
WHAT WE ARE LOOKING FOR:
- Bachelor's Degree required in Human Services (Master's degree preferred)
- 2 years' experience in addiction treatment (preferred)
- Experience in facilitating group therapy
- Specialty training preferred (MI, DBT, Art Therapy, MBSR, CPT CBT)
- Residential inpatient treatment experience (strongly preferred)
WHAT YOU WILL BE DOING:
- Overall coordination of a patients' care while in residential services, including integration with family services, sessions, communication with referral sources, assessment of patient progress, and communication to the clinical team regarding special treatment needs and procedures.
- Complete initial patient assessments (bio psychosocial assessment, patient goals and treatment planning).
- Utilize multi-disciplinary treatment team to determine appropriate level of care and identify behavioral health needs that may require external placement outside of Brookdale Premier Recovery.
- Facilitate group therapy, and individual counseling sessions.
- Document treatment plans, progress notes, treatment plan updates, case consultations, phone calls, and discharge summaries in the patient medical record.

SALARY RANGE (depends upon prior work experience and education) = 45K to 62K
