What are the responsibilities and job description for the MDS Coordinator position at Brookmont Healthcare and Rehabilitation Center?
```Duties```
- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for all residents in accordance with state and federal regulations
- Collaborate with interdisciplinary team members to ensure accurate and timely completion of assessments
- Review medical records and conduct interviews with residents, families, and healthcare providers to gather necessary information for assessment completion
- Ensure that all assessments are accurately coded and documented in the electronic health record system
- Monitor and track changes in residents' conditions and update assessments as needed
- Provide education and training to staff regarding MDS assessment process and documentation requirements
- Participate in quality improvement initiatives to enhance the accuracy and completeness of MDS assessments
```Qualifications```
- Bachelor's degree in nursing or a related field (preferred)
- Current RN/ LPN license (required)
- Strong knowledge of anatomy, medical terminology, and clinical documentation requirements
- Proficiency in medical coding, including ICD-10 coding system
- Experience with electronic health record systems such as Athenahealth, eClinicalWorks, Epic, etc.
- Familiarity with clinical documentation improvement processes
- Excellent communication and interpersonal skills
- Detail-oriented with strong analytical and problem-solving abilities
