Position Summary : The Patient Experience Coordinator engages the patient and caregiver in ongoing meaningful dialogue to establish and maintain a positive and supportive relationship with the goal of ensuring patient & family satisfaction. The Coordinator works collaboratively with our clinical teams and care partners in the community to overcome barriers and ensure the patient receives all ordered care and services in a timely manner. The coordinator investigates, resolves, documents and reports concerns and compliments to home health leadership and appropriate team members. The Coordinator supports our telehealth department and re-hospitalizations initiatives by providing patient follow up either by phone or in person as assigned. The Coordinator acts as the patient satisfaction ambassador by recommending and leading initiatives to achieve superior patient satisfaction outcomes and takes the primary role in establishing a culture of service within the agency.

- Maintains up-to-date tracking of all loaned equipment and devices ensuring equipment is returned in good condition; notifies Director of Nursing or Regional Administrator if equipment is lost, stolen, or returned in poor condition.
- Responsible for ordering, receiving, and dispensing of supplies for patient and staff use.
- Keys supply orders into the materials management module in Meditech within 48 hours of receipt supporting payment to the vendors and inventory control
- Keys supply charges into Oracle within 48 hours to ensure the charges are added to the billing claims.
- Ensures all other purchase orders and invoices are processed timely and in accordance with established processes and procedures.
- Responsible for proper storage of medical supplies and rotation of all goods (First In/First Out (FIFO)), including pulling damaged goods, removal of obsolete supplies as indicated, and informing the Manager as necessary.
- Ensures the accurate management and control of the Home Health supply inventory control systems.
- Accurately records all filled supply requests in Oracle.
- Orients new clinical staff to supply procedures and ensures that car stock inventories for new field staff readily available.
- Ensures that lab supplies, accompanying paperwork and required forms are always available and current.
- Ensures the organization and cleanliness of the supply area is maintained in survey ready status at all times.
- Ensures that supplies of clinical paperwork packets are available at all times.
- Communicates directly to the printing vendor to replenish paper supplies.
- Coordinates work to achieve maximum productivity and efficiency; recognizes and establishes priorities.
- Demonstrates appropriate judgment, requesting assistance as needed.
- Is highly accountable for quality, quantity and timeliness of work
- Maintains a pleasant and professional demeanor at all times ensuring that behavior is consistent with organizational values and policies.
- Works cooperatively with all staff & departments in support of agency needs and goals.
- Demonstrates knowledge of home health regulations and standards.
- Provides back up assistance to the Administrative Assistant as needed
- Accepts other duties as assigned willingly and pleasantly .
Job Qualifications:
- High School Diploma or GED, college degree preferred
- General knowledge of medical terminology, supply management, central supply
