What are the responsibilities and job description for the Licensed Practical Nurse (LPN), Brooks Center for Care Transitions, Full-time position at Brooks Rehabilitation?
The LPN for BCCT performs nursing functions under the direction of the Physician Clinic Manager, follows the plans of care, participates in care planning, and performs nursing actions in accord with hospital policy and state law. Responsibilities :
- Engage in regular telehealth visits within the first two weeks post-discharge, followed by scheduled calls over a 30-day period to ensure smooth transitions and address rehabilitation needs.
- Assist in therapy guidance, transitioning between home health and outpatient services, and facilitate communication with primary care physicians to optimize patient recovery.
- Document observations and care given in accord with hospital process according to hospital policies and procedures.
- Utilize home-based monitoring systems to track patients' vital signs, including blood pressure, oxygen saturation, weight, and pulse data, ensuring timely interventions when necessary.
- Provide virtual consultations to deliver medical advice, monitor patient progress, and adjust care plans as needed, enhancing accessibility and convenience for patients.
- Conduct telehealth sessions to evaluate and treat patients with complex conditions, such as chronic pain, and educate therapists in applying psychologically informed physical therapy skills.
- Maintaining accurate and confidential patient records obtained through telehealth interactions, ensuring compliance with healthcare regulations and standards.
- May perform other duties as assigned. Qualifications :
- Graduate of an approved school of practical nursing
