Job Summary
As a Registered Nurse in Home Care, you will provide high-quality nursing care to our residents according to established policies and procedures. Your expertise will be instrumental in developing and implementing individualized service plans that cater to each resident's unique needs. You will work closely with our interdisciplinary team to ensure seamless care delivery and exceptional outcomes.
Key Responsibilities
- Partnership and Collaboration:
- Collaborate with the initial evaluation team to assess residents' needs and develop comprehensive service plans.
- Communicate changes in residents' conditions effectively.
- Care Delivery:
- Implement and follow individualized plans of care developed in conjunction with residents, families, physicians, and authorized practitioners.
- Deliver high-quality nursing care according to acceptable standards and the plan of care.
- Professional Development:
- Participate in care conferences and other resident-related meetings to ensure optimal care coordination.
- Contribute to the orientation and training of agency nurses and Aide staff development activities.
Requirements
- Experience:
- Minimum 6 months' experience as an RN in a home care or similar setting.
- Preferred experience in caring for seniors and/or those with cognitive impairment.
- Skills:
- Basic computer skills, including proficiency in Microsoft Office, internet, and web applications.
- Experience with electronic medical records preferred.
