Social Worker (CASTLE ROCK, COLORADO)
Brookside Inn, a family-owned skilled nursing and rehabilitation facility in Castle Rock, is seeking a compassionate and conscientious full-time Social Worker to join our team of qualified individuals. Brookside Inn offers a small caseload for social workers with a total of 4 social workers for 120 bed licensed facility. We are looking for a professional who is committed to the highest standards of integrity, quality care, and respect for our residents.
Rate of Pay
$58,000-62,000 annually depending on experience

Position Summary:
The Social Worker provides essential social services to residents and their families, addressing their psychosocial, emotional, and practical needs. This role involves conducting assessments, developing care plans, providing counseling and support, advocating for resident rights, and collaborating with the interdisciplinary team to ensure comprehensive and individualized care.
Responsibilities:
- Conduct comprehensive psychosocial assessments and develop detailed social histories for each resident, contributing valuable insights to the interdisciplinary care team and resident care conferences.
- Collaborate with the interdisciplinary team to develop and implement individualized, measurable, and realistic care plans that optimize social service delivery and enhance residents' quality of life.
- Actively participate in the development, implementation, and regular reassessment of each resident's comprehensive care plan, ensuring it addresses their evolving social, emotional, and psychological needs.
- Provide compassionate support and counseling to residents and their families, addressing adjustment challenges related to long-term care placement, grief and loss, family dynamics, and other psychosocial needs. Maintain regular contact to ensure ongoing support and address emerging concerns.
- Advocate for resident and family rights and ensure their voices are heard in all aspects of their care.
- Facilitate communication and problem-solving between residents, families, and staff.
- Develop and maintain strong working relationships with community health, welfare, and volunteer organizations to cultivate collaborative partnerships that enhance access to resources and support services for residents and their families.
- Manage discharge planning, ensuring a smooth transition for residents returning home or to other care settings.
- Maintain accurate and timely documentation of all social work interventions.
Qualifications
• Degree Requirements: Bachelors or Master in Social Work, Human Services, Psychology, Gerontology, Counseling, Sociology or other related field
- 2 Years Long-term care experience (preferred)
- Responsible, patient, and compassionate, Excellent customer service skills
- Knowledge about Medicare and Medicaid (preferred)
- Demonstrate critical thinking, presentation, and organizational skills.
- Ability to assess residents and relate information regarding resident’s condition.
- Basic computer and typing skills
- Must have good interpersonal communication skills, both verbal and written.
- Ability to communicate and function productively on an interdisciplinary team.
- Must be able to read, write, speak, and understand the English language
- Ability to make independent decisions when circumstances warrant such action
- Demonstrate dependable, regular attendance
- Must be able to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.
- Ability to communicate policies, procedures, regulations, reports, etc., to personnel, residents, family members, visitors, and government agencies/personnel
- Must perform proficiently in all competency areas including but not limited to: patient care, patient rights, and safety and infection control.
