Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have experience in geriatrics and a strong understanding of dementia care, providing essential support to seniors and their families. This role involves working closely with patients in nursing homes and outpatient settings, ensuring they receive the care and resources they need to thrive.
Duties
- Conduct thorough patient assessments to determine individual needs and develop appropriate care plans.
- Provide counseling and support to patients and their families, addressing emotional, social, and psychological challenges.
- Collaborate with healthcare professionals to coordinate services and ensure comprehensive patient care.
- Maintain accurate medical records in compliance with HIPAA regulations, ensuring confidentiality and security of patient information.
- Advocate for patients’ rights and access to necessary services within the healthcare system.
- Educate patients and families about available resources, including community services for senior care.
- Stay informed about current best practices in dementia care and geriatric social work.
Qualifications
- Bachelor’s degree in Social Work (BSW) or related field; Master’s degree preferred.
- Experience in geriatrics, dementia care, or related fields is highly desirable.
- Strong knowledge of patient assessment techniques and care planning processes.
- Familiarity with medical records management and understanding of ICD-9 coding is a plus.
- Excellent communication skills, both verbal and written, with the ability to build rapport with patients and families.
- Ability to work independently as well as part of a multidisciplinary team.
- Commitment to maintaining confidentiality and adhering to ethical standards in social work practice.
