In this role, you will:

• Provide ongoing support to people in becoming active participants in every aspect of their lives (i.e., self-care routines, health, safety, household chores, socialization and overall well-being) and to ensure that a high quality of life is maintained.
• Ensure a safe and clean home by providing support to people to assist them with the completion of household chores and by participating in said household chores as needed.
• Support people in preparing meals by teaching concepts such as diet adherence, portion control and cooking skills; as well as assisting people with medication administration according to the AMAP policy and procedures.
• Implement and document all treatment plans and other agency related materials to assist people in achieving independence in their selected valued outcomes and to ensure their health, safety and well-being.
• Provide transportation to people supported for medical appointments, shopping, recreational outings and any other transportation needs.
• Mentor new Direct Support Staff by demonstrating supportive routines and by reviewing training manuals to ensure new employees are sufficiently groomed to complete responsibilities.

To succeed, you’ll need:

• Valid and Qualified NYS Driver’s License
• HS Diploma or equivalent preferred
• Ability to communicate and understand written and oral directives/information
• Physical aspects may include but are not limited to lifting and transferring of people (children and/or adults)
• Acting as a second support staff during transportation as needed, inclusive of wheelchair vehicle/lockdowns
• AMAP/First Aid/CPR/SCIP-R Training preferred
• Minimum one (1) year experience working with intellectual and other development disabilities preferred
• Passion to help others and a great personality
• Good verbal and written communication skills
