The Functional Job Description offers a useful list of the job responsibilities and expectations for the position of Community Based Program-QMHP. This list may be revised/updated at any time. Individuals in this position report directly to the specified Community Based Program Manager. The description does not supersede the direct supervisors’ directives/expectations or Brothers’ Keeper Inc. policies and procedures.
· Reports directly to Program Manager.
· Provide individual counseling/family counseling (when appropriate) and crisis intervention to eligible participants.
· Provide crisis stabilization service to participants and family members.
· Establish and maintain positive relationships with other agencies involved in the care of participants.
· Promote the program in a positive manner to elicit referrals and maintain census and growth of the program.
· Case Management and collaboration with other mental health professionals linked to participant
· Receive referral and provide telephone support for emergency crisis calls.
· Individual service planning through development and implementation
· Prioritizes safety first
· Identify and utilize techniques, interventions, and strategies to enhance participants’ strengths, life and social skills, and behaviors
- Maintain an accurate schedule counseling sessions to be reviewed at any time.
· Submit a copy of all Incident Reports to your direct supervisor.
· Ensure that every Incident Report is completed immediately after the incident and submitted to the direct supervisor or Director the same day.
· Complete all Progress Notes using sound clinical documentation and abiding by all agency, Medicaid and licensure standards within 48 hours of each session.
· Be available for 24 hour Emergency Response.
· Constantly monitor and document participants’ progress towards completion of treatment goals and objectives.
· Participate in monthly group supervision and weekly individual supervision with the specified clinical supervisor.
· Complete all assessments, bio-psychosocial reports and individual Service Plan on or before the due date.
· Attend all annual mandatory agency trainings (Handle with Care, First Aid & CPR, Human Rights, HIPAA, etc.).
· Identify clinical areas that require strengthening and seek out opportunities to attend training in such areas.
· Other duties as assigned.
· Conduct initial assessment and submit within 48 hours to CCC Plus/Medallion 4.0.
· Collaborate with LMHPs to determine the client’s diagnosis.
Minimum Qualifications
Qualified applicants will possess a master’s or bachelor’s degree in human services or related field verified by an official transcript from an accredited college or university. Applicants must be in good standing with the Virginia Board of Counseling.
Job Types: Full-time, Part-time
Pay: $20.00 - $24.00 per hour

- 8 hour shift
- Choose your own hours
- Monday to Friday
- Weekends as needed
