Per Diem Days

The licensed speech language pathologist is responsible for providing speech and language therapy interventions to patients/clients in an outpatient setting as ordered by the referring physician.

We are always looking for positive, dependable, caring individuals to join our Nursing team to meet the growing demand in our community for quality care. Speech Language Pathologist/ SLP duties include:
- Performs a comprehensive evaluation and reevaluation of patient in regards to speech language therapy needs upon referral in a timely manner using tests and measurements of speech, language, swallowing, and social or cognitive communication as appropriate. Based on the evaluation findings, a treatment plan will be established consisting of short and long term goals. Reevaluations of patients will be performed at least every 30 days as stated by current state regulations.
- The speech language pathologist shall work with members of the interdisciplinary team in an effort to develop a treatment plan as appropriate. Findings from the initial evaluation shall provide a platform for a discussion with the patient and/or family members in regards to the frequency of treatments and expectations in accordance with the prescription from the referring physician.
- The speech language pathologist shall provide sessions consistent with identified goals with focus on, but not limited to, the following areas:
- Comprehension
- Reading
- Oral motor functions
- Expressive language
- Written language
- Word retrieval
- Dysphagia
- Cognitive function
- Implementation of a home exercise program
- Documentation will be completed accurately and in a timely manner in accordance with state laws.
- Obtain necessary certification for implementation of vital stimulation to patients that would benefit from services
- A speech language pathologist will prompt necessary discharge from therapy services when appropriate.
- Short and long term goals have been met
- Progress is no longer expected secondary to functional plateau, noncompliance, etc.

Qualifications
- Must be currently registered/licensed in New York State. Must maintain an active license in good standing throughout employment.
- Must be ASHA certified.
