SUMMARY : The Behavioral Health (BH) Case Manager functions as a core member of a collaborative care team including behavioral health, primary care, and / or other clinical staff in a primary care setting. This position will serve as a liaison between BH providers and patients as the team works to achieve stated objectives and goals.
QUALIFICATIONS :
- Bachelor’s Degree in social work, psychology, counseling, with a minimum of one year case management experience or related field. Master’s degree preferred.
- Ability to communicate effectively in both oral and written form
- Ability to analyze, organize and prioritize work accurately while meeting multiple deadlines.
- Must be capable of exercising sound judgment when dealing with patients, staff and community at large.
- Ability to interface effectively with members of culturally and professionally diverse staff.
- Personal and professional qualities to work with the staff of the health center and with diverse populations.
