This position provides program oversight and supervision of the case managers in the CLTS (Children’s Long-Term Support) program. CLTS is a Medicaid program for children who are diagnosed with physical, emotional or cognitive disabilities. The goal of the CLTS Waiver Program is to support children with substantial needs, as well as their parents/guardians, by delivering services to promote the goal of every child living their best possible life now and into the future.

Supports CLTS case managers in all aspects including hiring, training, approval of timecards, performance evaluations, individual and group supervision and approval of training.
 
Provides case managers educational opportunities to ensure they are well versed in a variety of disabilities and the challenges CLTS families face.
 
Coordinate programmatic support and participant needs requests with Child and Family Program Specialist at the Bureau of Children’s Services and ensure a strong partnership with the Wisconsin Department of Health Services.
 
Assures the application of client rights, grievance and dispute resolution processes and procedures. Serves as the program contact in responding to consumer related complaints and grievances. 
 
Approves authorizations for purchase of services and items in line with service plans. Ensure the Deciding Together model is being implemented with families when making purchasing decisions.
 
Builds contracted services pool through educating community partners on CLTS and CCOP funding. 
Monitors and supports contracted service agencies to assure high quality and the appropriate extent of services provided.
 
Facilitates the establishment of a clear program vision, including planning and presentation of strategic annual goals, objectives and outcome measures; assures staff and contracted agent familiarity and performance in relationship to goals and monitors regularly to determine further action steps needed.   
 
Ensures program and staff compliance with applicable county, state and federal guidelines related to the provision of service. Facilitates annual Metastar compliance review process.
 
Coordinates and facilitates a quarterly Brown County Child and Family Advisory Board, as oversight for the county CCOP (Children’s Community Options Program) funding.
 
Maintains awareness of federal, state, county and local laws and regulations that impact funding and service delivery.
 
Facilitates collaboration and maintains positive working relationships with Brown County support staff including finance, contracts and purchasing. 
 
Reports data and program activities to the Health and Human Services Director, Community Services Division Administrator, Behavioral Health Manager, County Boards and Committees as requested.
 
Facilitates weekly staff meetings and monitors overall unit performance.
 
Functions as part of treatment teams/multi-disciplinary teams to ensure continuity of care.

ComputerEducation and Experience:
 
Bachelor's Degree (Master's Degree preferred) from an accredited university or college in Social Work, Human Services or a related field; plus three years’ experience in related case work including prior supervisory training and experience; or any equivalent combination of education, training and experience which provides the necessary knowledge, skills and abilities. Prior supervisory experience preferred.
 
Licenses and Certifications:
 
Valid Wisconsin Driver's License
State Social Worker Certification preferred
Newly hired staff must complete and pass DHS approved Waiver Basics online training course and Mandated Reporter Online Training as well as becoming a certified Wisconsin Children’s Functional Screener.

Knowledge of case management principles and methods, family dynamics, human development and behavior.

 

Knowledge of dynamics involved in community-based care for persons with long-term care

needs.

 

Knowledge of social work assessment principles and techniques.

 

Knowledge of client rights, grievance and dispute resolution processes and procedures.

 

             Knowledge of state and federal laws, codes, procedures and policies pertaining to Medicaid and Wisconsin Long-Term Care program delivery.

 

Knowledge of community resources.

 

Knowledge of needs and dynamics of those with all types of disabilities.

 

Ability to establish priorities for service coordination and to consider alternative treatment plans.

 

Ability to work independently.

            

Ability to use receive and provide feedback.

 

Ability to be decisive and to make sound decisions under the pressure of urgent situations.

 

Ability to plan and organize work to meet deadlines and to make effective use of time.

 

Ability to establish and maintain positive relationships with consumers and their family members, staff, contracted providers, and the public. 

 

Ability to communicate effectively both orally and in writing.

 

Ability to work the required hours of the position, which is 40 hours weekly, with core hours being 8:00am-4:30pm..

 

PHYSICAL DEMANDS:

 

Lifting 20 pounds maximum with frequent lifting and/or carrying of objects weighing up to 10 pounds.

 

Intermittent standing, walking and sitting; occasional driving.

 

Field work requiring face to face and collateral contacts located in home and community-based settings. 

 

Using hand(s)/feet for repetitive single grasping, fine manipulation, pushing and pulling, and operating controls.

 

Occasional bending, twisting, squatting, climbing, reaching, and grappling.

 

Distinguishing sounds at various frequencies and volumes.

 

Distinguishing people or objects at varied distances under a variety of light conditions.
