The Bryan County Family Connection is now accepting applications for a Full-Time Case Manager.
This position is to establish baseline needs assessments; provide case management and coordination of services; provide intake services; maintain records; and provide a variety of administrative duties to facilitate the effectiveness and efficiency of the program.
ESSENTIAL FUNCTIONS
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
Establish baseline needs assessment for each client and his/her family. Identifies needs for supportive services by observation of behavior and review of records. Listen carefully to concerns and ask effective questions to assist individual in expressing true feelings. Assist families to understand the case plan and program goals. Advise families and of other resources and make appropriate referrals as needed with supervisors’ approval.
Provide a variety of case management and coordination of services. Schedule visits with individuals and/or families as necessary. Assist with attendance to required sessions and appointments as specified in service plan. Establish and maintain effective working relationships with human service delivery agencies as appropriate.
Provide intake services as delegated by receiving referrals from Bryan County Family Connection Executive Director and/or Program Coordinator. Collect information and determine validity of referral in accordance with established procedures. Ascertain place of residence and the need for individualized plan. Collect data as required by Bryan County Family Connection Annual Operating Plan.
Maintains records on individuals/families enrolled in the program, contacts, case management services, ensure effective follow-up and provide data for required reporting. Maintain accurate up-to-date records of all assessments, plans, interventions, family involvement, outside agency communication, surveillance activities and other contacts. Write concise, accurate and organized progress notes and other case documentation. Prepare required documents, case forms, and other materials in compliance with laws, rules, policies and procedures. Maintains confidentiality of enrolled records in accordance with state law.
Provide a variety of administrative duties to facilitate the effectiveness and efficiency of the program. Develop and maintain and in-depth knowledge of departmental policies, state laws, rules and procedures as they relate to the job duties and the supervision program through regular review of documents and attendance at training events. Represent the program and the Family Connection Board in a professional manner by speaking to community groups, other agencies and related organizations about the Case Management Program. Participate actively in supervision and staff development activities by regular attendance at staff meetings and conferences.
Attend Bryan County Family Connection monthly collaborative meetings and other meetings pertinent to the position.
Punctuality and attendance are essential.
Performs other related duties as required.
MINIMUM QUALIFICATIONS: High School Diploma or GED required; preferred undergraduate major at a four (4) year college or university in human services delivery program, and/or work experience and/or field practicum, internship in any related human service field. Must possess and maintain a valid Georgia Driver’s License, own transportation, vehicle insurance and registration. Must be at least 21 years of age. Must clear the mandatory Criminal Background Check and Drug Screen.
