- Active California RN License upon hire
- Graduate of an accredited school of nursing
- At least 3 years of clinical experience in an HMO, hospital, medical group or clinic setting
- Bachelor's or Master's degree in Nursing or a related field
- Case Management Certification upon hire (CCM, ACM)
- Specialty Certification in Oncology, Rehabilitation, Pediatrics, Transplant, or Wound Care upon hire
- 3 years of experience in utilization management or case management
- Case Management & Discharge Planning
- Strong Communication & Coordination Abilities
- Knowledge of Healthcare Regulations & Resources
