Title: RN Behavioral Health Case Manager - Home Health
Status: Full-time, Contract-to-Hire
Assignment: 6-Month to 12-Month Contract Position
Pay Rate: $33.63 to $43.65 per hour, depending on experience
Location: :Western Cuyahoga, Ohio
Travel: Mileage Reimbursement at Standard IRS Rate
Visa Sponsorship: Not supported

The Behavioral Health RN's primary role is to function independently in providing comprehensive care to patients who are experiencing mental illness - both acute and chronic disease states.

Minimum Qualifications:

• • 2 years of experience with Behavioral Health, Mental Health, and/or Psych Patients within the last 3 years
• • Associate's Degree in Nursing, BSN is preferred
• • State of Ohio Registered Nurse license
• • Mental Health requires initial approval/certification by Medicare fiscal intermediary (Palmetto)
• • BLS Certification
• • Valid Driver's License

• • Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds.
• • Ability to work in a complex institutional structure requiring significant collaboration.
• • Strong planning and organizational skills.
• • Demonstrated ability to work well under pressure, within strict time parameters, and with strong attention to detail.
• • Ability to work independently with minimal supervision.
• • Demonstrated ability to maintain strict confidentiality.
• • Bilingual a plus
