Position Overview
- Role: Nurse Practitioner (NP) or Physician Assistant (PA)
- Location: Central Maine – Topsham and Rockland Area
- Schedule: Full-time (4 or 5 days per week) with no weekends, holidays, or overnight shifts
- Compensation: $70-$75/hour (W-2 Full-Time Employee or 1099)
- Work Model: Onsite care with some telemedicine coverage
- Provide comprehensive geriatric medical care across assisted living, long-term care, and memory care settings.
- Develop and oversee personalized health plans tailored to residents’ needs.
- Participate in telemedicine visits to ensure seamless care access.
- Collaborate with interdisciplinary care teams to optimize health outcomes.
- Utilize remote patient monitoring (RPM) tools for proactive care and early intervention.
- Maintain detailed and timely documentation in the Electronic Health Record (EHR) system.
- Active NP or PA certification with licensure in Maine.
- Experience in geriatric care or a strong interest in specializing in this field.
- Ability to work independently and collaboratively within a care team.
- Comfortable using technology tools and remote monitoring devices.
- Strong diagnostic and clinical assessment skills with a focus on patient-centered care.
- Excellent communication skills with patients, families, and team members.
