PRP Direct Care Staff/Counselor
Position Overview:
The PRP Direct Care Counselor provides weekly sessions to assigned clients in the community, home, and office. The PRP Direct Care Counselor is responsible for assessing clients’ needs and capabilities while supporting their individualized rehabilitation plan goals (IRPs).

Specific Responsibilities:
- Provide a minimum of 4-6 direct service sessions per client, per month as planned by the rehabilitation team (PRP worker and Rehab Specialist) and client.
- Provide therapeutic activities and opportunities that encourage and teach healthy social interactions, behavior modifications, age-appropriate self-care skills, independent living skills, and practice utilization of coping skills.
- Help clients achieve their goals by coaching, training, and supporting linkages to community resources.
- Transport clients to/from clinics and community events.
- Maintain contact with client, client's family and treatment team.
- Facilitate ongoing clinical collaboration with referring and treating clinicians and your direct supervisor to ensure a continuum of care for clients.
- Assess clients’ needs and develop IRPs that implement rehabilitation activities based on their needs as outlined in their individual treatment plans. Review every 3-6 months.
- Comply with CARF/COMAR/HIPAA/State compliance regulations, and EMR and uphold the 48-hour documentation policy. Other duties as assigned
Expectations:
- Attend supervision with a supervisor monthly or as requested.
- Attend and participate in trainings, including monthly PRP Team Meeting
- Notify supervisors of all crisis situations to help with decision making
- Respond to communication (phone, email, text) from clients and families, referral sources, and BTST staff within 24 hours or the next business day.
- Maintain working knowledge of information dispersed via notifications such as memos, formal notices, and e-mails from all levels of management.
WHAT YOU NEED:
- Ability to communicate with compassion and empathy
- A passion for making an impact on clients and the community
- Excellent in both written and verbal communications skills.
- Sensitivity to and understanding of cultural diversity
- Bachelor’s degree in Social Work, Psychology, or health-related field, or proof of completion of thirty (30) hours, or the equivalent of college credit toward a bachelor’s degree in a health-related field, or one (1) year of work experience in a supervised mental health setting.
- Valid, unrestricted driver’s license and reliable transportation.
