We are hiring a Short-Term Post-Hospitalization (STPH) Navigation Manager to join our team. As a Navigation Manager, you will play a pivotal role in bridging hospital care and community-based support for individuals navigating behavioral health and substance use challenges in Marin County. You will emphasize system navigation and care management by supervising System Navigators, coordinating services, and maintaining effective communication between clients, families, and community resources. You will support a "closed loop referral" approach, ensuring client referrals to services are tracked and completed to promote wellness and reduce barriers to recovery. By fostering a culture of client empowerment and community integration, you will help reduce hospital readmissions, improve service coordination, right level of care access at the right time and elevate client outcomes.

 

WHAT YOU WILL DO:

 

ADMINISTRATIVE

  • Assist with operational and administrative day-to-day activities of the Short-Term Post-Hospital Navigation program, ensuring smooth program functioning and adherence to strategic goals.
  • Develop and implement workflows, procedures, and documentation systems to support no wrong door, effective care coordination and a "closed loop referral" process.
  • Work with Program Director and the Finance team to prepare budgets within assigned program(s), monitor revenue and expenses to operate within budgets assuring that financial procedures and controls are adhered to by program staff.  Approves program expenditures in line with budgets in accordance with authorized authority.
  • Track and report program performance metrics, including outreach, client service outcomes, referral completion rates, and overall program effectiveness.
  • Serve as the primary liaison between internal teams and external partners to facilitate timely communication and service delivery.

 

STAFF MANAGEMENT

  • Supervise and provide leadership to System Navigators, including hiring, onboarding, and ongoing professional development.
  • Facilitate regular team meetings, case consultations, and individual supervision to enhance staff performance and morale.
  • Assist in the development and implementation of training materials, focusing on effective navigation strategies, care coordination best practices, and compliance with regulatory requirements.
  • Assist with the evaluation of staff performance, provide constructive feedback, and implement strategies for improvement where necessary.
  • Assist with coordination and management of staff schedules to ensure adequate coverage and meet the program’s operational needs.

 

PROGRAM QUALITY AND COMPLIANCE

  • Ensure that all program activities meet legal, regulatory, and contractual requirements, including compliance with CALAIM Enhanced Care Management and Community Supports standards.
  • Assist with regular audits of program documentation, client records, and service delivery.
  • Aid with design and oversee quality assurance activities, including reviewing care plans and tracking outcomes to ensure program fidelity and client satisfaction.
  • Participate in organizational efforts to improve service delivery, including data collection, evaluation, and reporting of program outcomes.
  • Support the mission and objectives of Buckelew Programs by maintaining the privacy and confidentiality of information, protecting the organization’s assets, and adhering to applicable federal, state, and local laws and regulations.

 

CLINICAL DUTIES

  • Oversee, assist, monitor and provide direction to staff in the planning, development, reviewing, and implementation of client services and record keeping. 
  • Participate in the development and implementation of client-centered care plans, ensuring that they address clinical and non-clinical needs, including Social Determinants of Health (SDOH).
  • Provide guidance to System Navigators in managing complex cases, offering strategies for engaging clients and resolving barriers to care.
  • Monitor the "closed loop referral" system to ensure timely and effective completion of client referrals to external services.
  • Ensure culturally sensitive service delivery, accommodating client preferences and language needs.
  • Provide crisis intervention and support as needed, coordinating with appropriate providers to resolve emergent issues.
  • Provide direct services and support to individuals receiving community support and enhanced care management services.
  • Ensure documentation, screening is completed and meets CALAIM regulations and requirements when delivering services.

 

Community Outreach & Engagement

  • Represent Buckelew Programs in community meetings, task forces, and public forums, advocating for the needs of underserved populations and promoting program services.
  • Build and maintain collaborative relationships with hospitals, behavioral health providers, housing agencies, and other community partners to enhance care coordination and referral pathways in order to provide peer and families and consumers with appropriate and up-to-date resources and support.
  • Develop and implement outreach strategies to increase program awareness and connect clients with essential resources including Partnership Health generated outreach lists.
  • Explore and maintain community resources
  • Represent client and family interests in various community committees and task forces.


WHAT YOU WILL BRING:

 

Education and Experience:  Master’s degree or Bachelor’s degree in social sciences, Certified Drug and Alcohol Counselor encouraged though not required, Psychiatric Technician’s license, Registered Nurse, or Peer Support Specialist Certification; or Patient Navigation Certification; or Certification in Community Health, Advocacy, Drug and Alcohol similar discipline and/or equivalent work experience. Five or more years of experience in a behavioral health organization. Five plus years of supervisory experience required. Strong understanding of family and group dynamics. Must have strong organizational and crisis management skills, strong knowledge of documentation requirements, treatment planning, and assessment. Two or more years of experience with electronic health records (EHR) preferred. One or more years of experience in first responder related field, preferred. 

 

Professional:  Must be fluent in English. Bilingual English/Spanish preferred. Ability to work in an independent, organized, calm and reassuring manner using good boundaries and good judgment; to be reliable, dependable, and flexible; and to accept supervision, suggestions, and feedback. Ability to work within all the rules, regulations, and policies that govern agency compliance. Must have excellent oral and written skills, be organized, responsible, professional, able to work on many projects simultaneously, to be flexible, empathic, and able to relate to clients, other professionals, and the general community. Intermediate to advanced skills in MS Office (including Excel, Word, Outlook).  Must be a positive team player.  Must have and maintain a valid California driver’s license with an insurable driving record and insured personal car to use on the job. 

 

Physical Requirements:  Physically capable of the following intermittent activities: stretching, bending, kneeling, twisting, squatting, reaching above and below the shoulder, pushing, pulling, grasping, and lifting up to 30 pounds in weight. Physically capable of the following extended activities:  driving, writing, standing, typing, and sitting.
