We are currently seeking a full-time RN Case Manager to oversee an independent/assisted living facility. The RN who fulfills this role will be responsible for running the onsite clinic during operating hours, as well as completing home health care visits within the building, and other duties as assigned.
Responsibilities:
- Work with the physician for medication needs and changes to the plan of care.
- Manage other disciplines such LPNs, aides, therapy staff to give the best care needed for each patient.
- Ability to complete OASIS, recert, d/c, ROCs documentations within our EMR system (WellSky/Kinnser)
- Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
- Attend orientation, meetings, and CEUs.
