Key Responsibilities:
- Provide care wherever the patient calls home, focusing on delivering compassionate and effective therapy.
- Oversee and implement the PT plan of care as directed by the patient's physician and Physical Therapist.
- Collaborate with other disciplines to provide comprehensive care tailored to each patient's needs.
- Complete timely and accurate documentation within the Electronic Medical Records (EMR) system.
- Help reduce the risk of re-hospitalization by actively monitoring and adjusting the patient's plan of care as necessary.
- Provide education and counseling to family members about care, disease processes, and injury management, when appropriate.
- Participate in team meetings and Continuing Education Units (CEUs), occasionally attending meetings at the Cincinnati office/Blue Ash area.
- Must have a current, active Physical Therapy Assistant (PTA) license in Ohio, in good standing.
- Experience using EMR systems for documentation.
- A reliable automobile and current auto insurance are required.
- Current CPR certification.
- Strong ability to work independently, with excellent time management skills.
