Overview
As the Director of Nursing, you will work closely with the QAPI Director and Admin to ensure our agency delivers high-quality patient care and complies with all healthcare regulations. You will divide your time between fieldwork (patient care and assessments such as recertification, admission, discharge, etc.) and office duties (documentation, performance improvement initiatives, and compliance monitoring).

Examples of Responsibilities in Action:
Example 1: You may be asked to join the field team to assess a new patient’s care needs, ensuring that the care plan aligns with the patient’s desire, regulatory standards, and best practices.
Example 2: If there is an identified pattern of patient falls, ER, hospitalization or infections reported, you will help investigate the root cause, collaborate with the QAPI Director to create an action plan, and monitor its implementation.
Example 3: During your office hours, you will review patient care records, monitor compliance, and prepare reports that highlight areas for quality improvement or regulatory concerns.
Example 4: During field visits, you will spend at least 45 minutes and complete patient assessment with OASIS(Alora) tools, update physician plan of care (485), update the home health aide care plan and submit documentation for review while in the patient’s home. All documentation must be submitted same day as visits.
Experience
- Current Registered Nurse (RN) license in good standing.
- Previous experience in home care or nurse management preferred.
- Familiarity with EMR System and documentation review processes.
- Strong leadership skills with the ability to supervise nursing staff effectively.
- Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality.
- Excellent communication skills for effective interaction with patients, families, and healthcare professionals.

Pay: $64,000.00 - $75,000.00 per year
