We are seeking an Assisted Living/Memory Support Coordinator to join our community committed to delivering outstanding care to our residents. What You'll Do:
- Conducts scheduled care plan meetings in accordance with regulatory requirements. Documents changes in medical charts.
- Guides Assisted Living/Memory Support personnel in the development of personal and professional objectives and in the setting of program goals consistent with those established by the Assisted Living/Memory Support departments.
- Conducts community tours for admission prospects and visitors; answers telephone inquiries; assist with admissions process in the absence of the Director.
- Participates in pre-admission evaluation of prospective residents; provides feedback as required.
- Directs Assisted Living/Memory Support personnel in fulfilling their responsibilities for resident care.
- Provide high-quality care and assistance, including bathing, feeding, oral care, dressing, and mobility support.
- Accurately document Activities for Daily Living (ADLs) and other relevant information.
- Assist with activities.
- Accurately performs I&O; checks vital signs and weight; performs sugar and acetone urine testing, specimen collection and post-mortem care.
- Intervene professionally in resident disputes or disruptive behavior, reporting incidents to the charge nurse.
- Perform other tasks in support of your team including: staff nursing station and answer phones as needed; assist in dining room with meal serving and feeding as needed; and work schedules, assignments and units as requested.
- CPR/First Aid certified.
- High School Diploma (or G.E.D.) plus a minimum of 1 year prior experience as a nursing assistant or other equivalent training.
- Prior experience in in a direct care setting of an Assisted Living or Memory Support department.
- Ability to perform the physical requirements of the job including bending, stooping, twisting, turning, reaching, lifting, carrying, pulling, pushing, climbing, kneeling, walking, and standing over 75% of shift.
- Ability to exercise patience, tact, initiative, judgment, and confidentiality (following established guidelines).
- Ability to recognize changes in a resident's appearance, attitude, and condition.
