Description : POSITION SUMMARY :
This position integrates Health information coordination’s and New Patient Coordinator duties. The Health information coordinator / New Patient Coordinator is responsible for all new patient intake, registration processes, scheduling / coordinating patient for sliding fee and preparation / analysis of statistical reports for administration. Health Information coordinator duties also include but are not limited to a variety of tasks to ensure that faxed / mailed records are ready for clinical access and review pending scheduled appointments.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
- Perform intake and registrations for all new patients and coordinate all information with the New Patient Intake RN.
- Schedule new patient visits and provide EOM report on new patient enrollment / visit status.
- Facilitate all new patient needs with all programs available such as but not limited to, Sliding Fee, ACA programs.
- Receives faxed and mailed documents, checks for urgency and prioritizes; accordingly, scans and assigns documents electronically. Processes daily mail. Proper indexing / dates. Time management.
- Scans internal medical records and documents into ECW as needed.
- Updates medical records by assigning received documents to the proper patient file and assigning to the appropriate clinical staff person to review.
- Sends out faxes when needed for provider offices.
- Answers health information calls from outside offices / patients / emergency rooms / labs / DHHS / SSA / HEDIS reviews / TTR.
- Copies and mails records as authorized for outside providers, lawyers, etc.
- Responsible for general neatness of the health information / medical records room.
- Receives faxed and mailed documents, checks for urgency and prioritizes accordingly; scans and assigns documents electronically.
- Scans internal medical records and documents into ECW as needed.
- Updates health information records by assigning received documents to the proper patient file and assigning to the appropriate clinical staff person to review.
- Sends out faxes when needed for provider offices.
- Provides back up for Patient Services Representatives
- Brings any complex issues to the Practice Manager
- Participates in meetings and workflow redesign projects, as requested
- Performs other duties and tasks as assigned.
