What are the responsibilities and job description for the Health Home Care Manager position at Buffalo Federation of Neighborhood Centers?
Position Purpose: The Health Home Care Manager provides services to individuals living with a behavioral health diagnosis or multiple chronic health condition who reside in Erie County.
Primary Position Responsibilities
- Completes health home care management program screenings, intakes, discharges and all other requirements.
- Develops alongside the individual, a Plan of Care using the principles of person-centered planning that includes goals, preferences and strengths, functional needs, community supports, interventions and timeframes for improving the individual’s health.
- Maintains the Plan of Care with periodic reassessment of the individual’s needs, progress towards meeting their goal, and documents any changes needed.
- Assists individual’s receiving services and with applying for necessary benefits and entitlements.
- Provides guidance and services to individual as specified in the individual’s care plan, provides linkages and connections to health care services, psychiatric and counseling services and other services as needed.
- Completes Eligibility Assessments and Community Mental Health Assessments annually and as needed for those person’s enrolled in non-HARP and HARP programs.
- Must be available to transport individuals to appointments and/or community resources when needed.
- Schedule and assigned duties are subject to change based on the needs of the program and the individuals we serve we serve.
KNOWLEDGE AND SKILL REQUIREMENTS
- High School Diploma, 3 years of experience in a human services or healthcare setting working directly with people preferred.
- Must have a valid New York State driver’s license.
- Must have an automobile in good working condition.
- Demonstrated passion for working with people from all backgrounds and ages and providing person-centered care supported by excellent interpersonal and communication skills.
- Excellent written and oral communication skills with strong organizational skills.
- Fully proficient in Microsoft Office and experience with or ability to master electronic health records.
- Must be able to work independently.
