Position Overview:
As the Department Manager, you will be responsible for overseeing the daily operations of HHUNY. You will work with a team of care managers, providing personalized and effective care coordination to our clients. Your role will involve managing resources, implementing policies, and ensuring compliance with healthcare regulations. You will also collaborate with other departments and external partners to enhance service delivery and achieve organizational goals.
Key Responsibilities:
- Develop and implement departmental policies, procedures, and best practices to ensure high-quality service delivery.
- Ensure compliance with all relevant healthcare regulations and standards.
- Collaborate with other departments and external partners to coordinate care and improve client outcomes.
- Analyze data and generate reports to track departmental performance and identify areas for improvement.
- Serve as a point of contact for escalated client concerns, resolving issues promptly and effectively.
- Stay current with industry trends and best practices in care management and healthcare services.
Qualifications:
-Must have previous experience in Care Management role
- A Bachelor’s degree in Social Work, Psychology, Nursing, Rehabilitation, Education, Occupational Therapy, Physical Therapy, Recreation Therapy, Counseling, Community Mental Health, Child and Family Studies, Sociology, Speech and Hearing, or another Human Services-related field OR New York State Registered Nurse (RN) license and registration with a bachelor’s degree OR Credentialed Alcoholism and Substance Abuse Counselor (CASAC)
- Minimum of 3 years of experience in care management or a related healthcare field.
- Strong knowledge of healthcare regulations, care coordination, and case management practices.
- Ability to manage multiple priorities and work effectively in a fast-paced environment.
- Proficiency in using care management software and Microsoft Office Suite.
- Commitment to providing high-quality care and improving client outcomes.
Experience
-Providing direct services to people with physical chronical conditions, serious mental illness, developmental disabilities, or substance use disorders
or
Helping people with these conditions connect to a wide range of services to support them in the community
Job Types: Full-time, Part-time
Pay: $20.00 - $25.00 per hour
Expected hours: 40 per week
