Job Overview
We are seeking a dedicated and skilled Nurse Practitioner or Physician Assistant to join our healthcare team. This role requires a compassionate approach to patient interactions and a commitment to improving health outcomes through comprehensive assessment and treatment plans.
Responsibilities
· Collaborating with healthcare providers: The care coordinator advanced practitioner will collaborate with other healthcare providers, such as physicians, nurses, and social workers, to ensure that patients receive the best care possible. They will work with these professionals to develop treatment plans, coordinate care, and monitor progress.
· Managing patient care: The care coordinator advanced practitioner will be responsible for managing the care of patients with complex medical needs. They will review medical records, assess patient needs, and develop care plans that meet those needs. They will also ensure that patients receive appropriate referrals to specialists and other healthcare providers as needed.
· Coordinating care: The care coordinator advanced practitioner will coordinate care for patients across different healthcare settings, such as hospitals, clinics, and home care. Collaborate with Primary Care Physicians on the fly for complex patients with acute exacerbations. They will work with patients and their families to ensure that they understand their treatment plans and follow them correctly.
· Providing patient education: The care coordinator advanced practitioner will provide patients with information about their conditions, treatment options, and preventive care measures. They will help patients understand their treatment plans, and provide guidance on how to manage their conditions effectively.
· Monitoring patient progress: The care coordinator advanced practitioner will monitor patient progress, assess treatment effectiveness, and adjust care plans as needed. They will also evaluate patient outcomes and make recommendations for improvements to the care delivery system.
· Ensuring quality care: The care coordinator advanced practitioner will ensure that patients receive high-quality care that meets the highest standards of quality and safety. They will work to implement best practices and evidence-based care to ensure that patients receive the best possible care.
Minimum Qualifications:
· Experience caring for adults with serious illness is preferred
· Previous EMR experience, particularly epic, is preferred
· Must be able to perform any clinical or clerical duty as assigned skillfully; experience with IV starts preferred
· Must demonstrate strong verbal and communication skills including ability to present information in a group setting
· Must demonstrate the ability to handle stressful situations appropriately
· Must be able to work flexible hours as needed
