Program Summary:
The Long COVID Program at the Buffalo Urban League is designed to provide education, support, and resources to individuals experiencing Long COVID symptoms. The program collaborates with healthcare providers, community organizations, and public health departments to address medical, social, and emotional challenges associated with Long COVID, particularly among underserved populations.
Position Summary:
The Community Health Worker-Long COVID representative will work collaboratively with healthcare professionals, community organizations, and affected individuals to implement programs that educate, support, and assist those experiencing Long COVID symptoms. This position requires a compassionate, resourceful individual with a strong background in program coordination, community health, or case management, particularly in health-related fields and computer literacy.
Case Management and Support
• Develop, coordinate, and deliver educational workshops on Long COVID symptoms, treatments, and available resources.
• Collaborate with medical professionals and community organizations to design impactful programming and services.
• Identify gaps in services and create strategies to address them for individuals affected by Long COVID.
• Oversee the scheduling and facilitation of health education events, support groups, and one-on-one consultations.
Community Outreach and Support
• Conduct outreach to underserved populations to promote program services and improve access to Long COVID-related support.
• Serve as the primary point of contact for participants, addressing their concerns and guiding them to appropriate medical or social services.
• Advocate for the needs of participants by collaborating with local healthcare providers and organizations.
• Assist participants in navigating systems including Covid registry and follow-up care.
Administrative and Reporting Duties
• Maintain accurate records of participant interactions, program outcomes, service utilization, and data entry into the Electronic Medical Records (EMR) system.
• Develop and submit program progress reports and evaluations to agency.
• Ensure compliance with all state and federal guidelines for health program operations.
Collaboration with Medical Professionals
• Work alongside healthcare providers to support participants’ medical needs.
• Serve as a liaison between participants and healthcare systems to facilitate clear communication and care coordination.
• Stay informed on the latest research, treatment protocols, and guidelines related to Long COVID.
Qualifications
Required Qualifications:
• High School diploma or equivalent with a solid knowledge base in community service, public health, or program coordination, supported by relevant certifications or volunteer experience.
• Reliable transportation and a valid driver’s license.
• Strong communication and interpersonal skills to work with diverse populations and stakeholders.
• Proficiency in Microsoft Office Suite and familiarity with database management systems.
Preferred Qualifications:
• Associate or bachelor’s degree in public health, social work, or a related field.
• Experience in health education, patient advocacy, or working with individuals with chronic illnesses.
• Experience with public health initiatives and grant-funded programs.
• Multilingual capabilities are a plus.
