Bunny Home Care is seeking a charismatic and customer-focused Care Coordinator to join our dynamic team. If you thrive on building relationships, providing top-tier customer service, and making a meaningful impact, this is the role for you!
Responsibilities:
- Manage and coordinate patient care, ensuring a smooth and high-quality experience.
- Build strong relationships with clients, caregivers, and service coordinators.
- Handle last-minute caregiver call-outs and ensure proper coverage.
- Assist with onboarding, training, and compliance for new Caregivers.
- Conduct home visits for care management and quality assurance.
- Manage a caseload of 150 clients.
- Monitor key performance indicators (KPIs) and provide feedback for improvement.
- Address caregiver and client concerns with professionalism and empathy.
- Conduct in-field marketing and event participation to support business growth.
- Actively engage in outreach efforts to identify and attract new clients.
- Build partnerships with community organizations and referral sources.
- Represent Bunny Home Care at community events(Presentations), networking opportunities, and industry-related functions.
- Educate potential clients and their families about our services.
- Sign new cases and ensure a smooth onboarding process for clients.
- Conduct quarterly evaluations to maintain service quality (Remind Clients on our 5-star reviews for participation).
Requirements:
- Strong charisma and a positive, engaging attitude
- Excellent communication and customer service skills.
- Experience in healthcare, nursing, administration, or customer service is preferred.
- Strong organizational, problem-solving, and multitasking skills.
- Team player with a warm, compassionate demeanor, especially when working with elderly clients.
- Ability to maintain strict confidentiality.
- Proficiency in computer skills, including Microsoft Office, Google Workspace, and preferably Monday and HubSpot.
- Experience working with HHA is required.
- Bilingual skills are a plus – we value the ability to connect with diverse clients.
- Valid driver’s license and reliable vehicle required for traveling to client locations.
- Willingness to travel within a 45-mile radius to support client needs.
