What are the responsibilities and job description for the Social Services Coordinator position at Burcham Hills?
Burcham Hills is an upscale senior living retirement community located in the backyard of Michigan State University in East Lansing, MI. We currently have a vacancy for a Social Services Coordinator to work a Part Time schedule in our Center for Health and Rehab. In this position the Coordinator is responsible to work closely with clients, families and the interdisciplinary team to help clients transition to the next level of care. The social worker/discharge planner helps to remove barriers to the clients timely and safe transition and in addition to but not limited to the following duties:
- Assist in developing preliminary and comprehensive social service needs assessments and plan of care for residents to include identifying resident/client issues, needs and goals.
- Encourage residents/clients and family members to participate in the development and review of care.
- Interview residents and families to obtain social history.
- Evaluate social and family information to assist in determining plans for social treatment.
- Interpret and relay the social, psychological, and emotional needs of the resident/client and family to the interdisciplinary team.
- Provide consultation to members of the community in efforts to solve the needs and problems of the resident through the development of social service programs.
- Provide Medicare/Medicaid and financial assistance information to residents/clients and families as appropriate. Assist in obtaining resources from community social, health and welfare agencies to meet post d/c needs.

Qualifications: Licensed Bachelor of Social work or Gerontology, required. Minimum of one year experience in a fast-paced sub-acute, Long-Term Care or a Hospital Setting is preferred. Experience with MDS, Care Conferences and Discharge Planning is also preferred. Ability to read, write, speak and comprehend written and oral instructions in English. Proficient in Windows environment, MS Office and other data base programs.
