Summary
Person in this position will assemble patients’ health information, assures that patients’ initial medical charts and forms are complete, properly identified, signed, and that all necessary information is entered electronically. Clerk will also manage medical record scanning process for the inpatient and outpatient population.
Responsibilities
- Maintain and organize patient medical records in accordance with established protocols.
- Ensure the accuracy and confidentiality of all medical documentation.
- Assist healthcare providers with accessing patient records as needed.
- Update patient information in electronic health record systems.
- Coordinate with other departments to facilitate the flow of information.
- Respond to requests for medical records from authorized personnel.
- Pull charts as needed for special audits and peer review.
- Close discharged patient records within appropriate days of discharge per agency procedure ensuring all documentation is complete and all documents are signed and dated by the appropriate discipline.
- Identifies and informs supervisor of discrepancies if found related to the medical records functions requiring completeness and regulatory compliance, and if discrepancies are discovered develop immediate corrective actions to meet the state and federal regulatory requirements for accuracy and completeness.
- Performs other tasks and duties as assigned in an efficient, effective and cost-conscious manner, complying with all hospital policies, procedures and regulations.
Requirements
- Proven experience in a medical office or healthcare setting preferred.
- Strong understanding of medical terminology and record-keeping practices.
- Familiarity with electronic health record (EHR) systems is a plus.
- Excellent organizational skills and attention to detail.
- Ability to maintain confidentiality and adhere to HIPAA regulations.
