Job Summary:

Are you a dedicated Licensed Clinical Social Worker (LCSW) with a passion for pediatric care? We are seeking a skilled LCSW to join our care team. You will play a vital role in coordinating and managing the care of pediatric patients, focusing on their emotional and social well-being. The behavioral health care manager is a core member of the collaborative care team, including the patient’s medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other mental health providers.

LCSW Duties and Responsibilities:

  • Care Coordination: Collaborate with pediatric patients, their families, healthcare providers, and community resources to develop comprehensive care plans that address the unique emotional and social needs of each patient.
  • Assessments and Treatment Planning: Conduct thorough assessments of pediatric patients, including emotional and social factors, and develop individualized treatment plans in collaboration with the patient and their family.
  • Patient Advocacy: Serve as an advocate for pediatric patients, ensuring their emotional and social needs are met and their rights are protected throughout the care process.
  • Education and Support: Provide education to patients and their families regarding emotional and social wellbeing, coping strategies, and available support services. Offer guidance and emotional support to patients and families throughout their healthcare journey.
  • Referrals and Resource Management: Identify appropriate resources and make referrals for specialized services, therapies, and additional support systems as needed. Ensure efficient management of resources to optimize patient outcomes.
  • Collaboration: Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic care for pediatric patients. Maintain open lines of communication with physicians, nurses, therapists, and other healthcare professionals.
  • Documentation: Accurately document patient assessments, care plans, interventions, and outcomes in a timely manner, adhering to organizational and regulatory guidelines.
  • Focused Counseling: Provide individual and family counseling sessions to pediatric patients, focusing on their emotional, mental, and psychosocial wellbeing. Utilize evidencebased therapeutic approaches to address a range of issues such as anxiety, depression, trauma, grief, and behavioral challenges.

Behavioral Health Care Manager Duties and Responsibilities:

  • Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers
  • Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and followup care
  • Provide patient education about common mental health and substance abuse disorders and the available treatment options
  • Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment
  • Provide brief behavioral interventions using evidencebased techniques such as behavioral activation, problemsolving treatment, motivational interviewing, or other treatments as appropriate
  • Provide or facilitate inclinic or outside referrals to evidencebased psychosocial treatments (e.g. problemsolving treatment or behavioral activation) as clinically indicated
  • Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan (case reviews may be conducted by telephone, video, or in person)
  • Track patients follow up and clinical outcomes using a registry. Document inperson, telephone encounters in the registry, and use the system to identify and reengage patients. Registry functions is accomplished through an EHR build on a spreadsheet used in conjunction with an EHR or can be built into a standalone clinical management tracking system that may or may not be linked to an EHR
  • Document patient progress and treatment recommendations in EHR and other required systems shared with medical providers, psychiatric consultants, and other treating providers
  • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant who may need more intensive or more specialized mental health care
  • Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment)
  • Develop and complete relapse prevention selfmanagement plan with patients who have achieved their treatment goals and moving towards graduating from the caseload
  • Develop protocols for the Collaborative Care Model (CoCM) role

Training & Licensure Requirements:

The behavioral health care managers have specialized training and meet clinical licensure requirements to be able to provide brief psychosocial interventions such as problem-solving treatment.

Licensures and Educational preparation recommended:

  • Licensed Mental Health Counselor/Professional Counselor
  • Licensed Marriage and Family Therapist
  • Licensed Clinical Social Worker (e.g. LCSW)
  • Registered Nurse (BSN recommended)
  • Nurse Practitioner
  • Licensed Psychologist
  • Masterslevel licensure candidate/trainee (e.g. LMSW)
  • Minimum 4 years of clinical experience in social work, preferably in a pediatric or family care setting

Other Requirements:

  • Strong knowledge of pediatric healthcare practices, principles, and standards
  • Demonstrated ability to collaborate and communicate effectively in a team setting
  • Excellent communication and interpersonal skills, with the ability to establish rapport and build relationships with patients, families, and healthcare providers
  • Compassionate and empathetic nature, with a commitment to providing patientcentered care
  • Ability to work independently as well as collaboratively within a multidisciplinary team
  • Knowledge of community resources and support services for pediatric patients
  • Ability to maintain effective and professional relationships with patients and other members of the care team
  • Experience with screening for common mental health and/or substance abuse disorders
  • Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate
  • Ability to engage patients effectively in a therapeutic relationship, when appropriate
  • Ability to work with patients by telephone as well as in person
  • Experience with assessment and treatment planning for common mental health and/or substance use disorders
  • Working knowledge of evidencebased psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problemsolving treatment, behavioral activation)
  • Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role
  • Experience with evidencebased counseling techniques
  • Effective organizational and time management abilities, with the capacity to prioritize and manage multiple tasks in a fastpaced environment
  • Proficient computer skills and experience with electronic health record (EHR) systems

Physical Requirements:

  • Sedentary Work:

Exerting up to 10 pounds of force occasionally (up to 1/3 of the time) and/or a negligible Amount of force frequently (1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time but may involve walking or standing for brief periods. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

  • Hearing/Vision:

Hear and differentiate low volume sounds to make judgments regarding actions needed, i.e. hear alarm bells, verbal conversations, telephone voices and normal volume sounds. Close visual acuity is required (clear vision at 20 inches or less and ability to identify and distinguish colors) to perform activities such as preparing and analyzing data and figures, transcribing, viewing a computer terminal, and extensive reading.
