What are the responsibilities and job description for the Social Worker / Case Manager (Adult Protective Services) position at Burnett County?
This full-time case management position will provide services for Adult Protection cases. Duties include assessing the safety of vulnerable adults and the elderly including conducting welfare checks and providing initial screening and investigations of these referrals; determine and provide immediate services and providing appropriate referrals to community service providers. The position also locates resources as needed and may provide ongoing case management as needed. Intervention as outlined in Chapter 55 to include protective services and protective placements of an individual to assure safety and protection from abuse, neglect and exploitation.
This position will provide Access Intake duties on a rotating basis for the Behavioral Health Unit as well as provide after-hours crisis response between the hours of 4:00 pm and midnight.
Essential Functions of the Position:
- Maintains responsibility for Adult Protective Services referrals and investigations. Assessment of vulnerable adults and the elderly to ensure service needs are met and to protect individuals from abuse, neglect and exploitation. Provide initial screening and investigation on APS referrals and Welfare Checks; determine and provide immediate services and make appropriate referrals for services to community providers. Locate resources and provide on-going case management, as needed.
- Prepare court reports/documents for protective placements and all other documentation required by the Circuit Court. Testifies in court by providing expert testimony and preparing comprehensive reports. Represents the department professionally and in accordance with agency rules and expectations.
- Provide on-going case management on a limited number of APS cases. This includes: case management/ planning home visits, phone calls, face to face contacts in the home or community setting, and correspondence with community providers. Provide annual reviews for protective placements.
- Provide crisis follow up as needed/assigned.
Qualification Requirements:
- Bachelor’s Degree in Social Work, Psychology, Guidance and Counseling or closely related field. Current certification as a Social Worker in the State of Wisconsin is preferred, but not required.
- Two years of post-graduation professional case work at the type and level consistent with a County Human Services Social Worker.
- Ability to pass required background checks
- A valid Driver's license required.

- Ability to maintain confidential information.
- Ability to meet and deal with public and to establish and maintain an effective working relationship with staff and members of the public.
- Ability to exercise good judgment in approaching situations and making decisions.
- Ability to prepare and maintain clear and accurate records and reports.
- Ability to carry out directions and implement programs and policies set forth by governing committees.
