Job Description :
Join a dynamic healthcare team providing outpatient rehabilitation services directly to the older adult population in their homes or independent living facilities. As a Physical Therapist, you will evaluate and develop individualized treatment programs to help patients restore, enhance, and maintain their mobility and independence. You will collaborate with other rehabilitation professionals, refer patients to other disciplines as needed, and create safe, functional treatment plans in their home environments.
