What are the responsibilities and job description for the Direct Support Professional - Greenwood Day Program - Trainer/Driver PT 8am - 1pm M - F position at Burton Center?
Assists individuals with self-help skills (i.e. toileting, eating, etc.)
Implements facilitated plans.
Monitors individuals and completes monthly progress reports on all objectives.
Keeps daily payroll sheets of work accomplished.
Assists in the evaluation and placement of individuals for various jobs.
Assists workshop supervisor in any aspect of vocational training.
Transports individuals to and from Burton Center and on field trips.
Keeps a daily van maintenance sheet, mileage sheet, and accountability sheet.
Reports malfunctions immediately to transportation supervisor in writing.
Supervises individuals during preprogram breaks, lunch, and all special activities.
Assists with extra curricular activities.
Provide one on one care to individual if that level of care is required for position, following plan of care of the individual, to include, but not limited to feeding, toileting, etc.
Attends initial Burton Center staff orientation, in-services and workshops as appropriate to enhance job performance.
