Responsibilities
- Provide direct patient care and case management services
- Collaborate with healthcare providers to ensure coordinated and quality care for patients
- Conduct assessments, develop care plans, and monitor patient progress
- Maintain accurate and up-to-date medical records while adhering to HIPAA regulations
- Monitor vital signs and patient responses to treatment
- Coordinate transitions of care between different healthcare settings
- Participate in multidisciplinary team meetings to discuss patient care plans
Skills
- Knowledge of managed care principles and practices
- Familiarity with Level I Trauma Center protocols
- Proficient in medical records management
- Understanding of HIPAA regulations and patient confidentiality
- Ability to assess and interpret vital signs accurately
- Strong case management skills
- Previous experience working in a clinic setting would be advantageous
Job Type: Contract
Pay: $63.85 - $69.00 per hour
Expected hours: 40 per week
