What are the responsibilities and job description for the Health Navigator Specialist position at Butler County Health Department?
POSITION SUMMARY:
Under the general direction of the Senior Epidemiologist of Community Health Services, the Health Navigator Specialist will help connect community members within Butler County to various treatment and social services that are requested. The specialist will work in a variety of yet-to-be fully determined settings that may include local Hospitals, Emergency Departments, and Treatment Facilities/Centers where they will provide individuals in need with resources like naloxone, drug testing strips, educational information, and connect them to various potential treatment/medical and social services. Additionally, the specialist may assist the harm reduction department in other areas including the Syringe Service Program (SSP), naloxone distribution and education, and participating in various outreach activities and events.
ESSENTIAL FUNCTIONS:
- Develop and maintain relationships with local Hospitals, Emergency Departments, and Treatment Facilities/Centers where employee will be stationed throughout the work week during various days and times
- Facilitate referrals and warm handoffs to wraparound services in various settings
- Engage with individuals in need of services by providing them with compassionate and appropriate care, support, and education
- Help integrate naloxone and fentanyl test strips into various settings where stationed
- For each service encounter the employee will complete and fill out a Health Navigator Checklist and then be responsible for reporting and submitting the data
- Attend quarterly collaboration calls about the position with the Clark County Combined Health District (CCCHD) and the Ohio Department of Health (ODH)
- Have a comprehensive and well-rounded knowledge of Butler County treatment/recovery services, social services, and various medical services to be able to refer clients and patients to
- Ability to adapt to new work settings and be flexible in scheduling
- Willing to help staff Butler County Syringe Service Programs (SSP) and provide individuals with harm reduction services and educational resources as needed
- Ability to help the Harm Reduction Department in various ways when needed; ex; targeted outreach during overdose spikes, employee transitions or sick time, during important community events
OTHER DUTIES AND RESPONSIBILITIES:
- Work hours may vary, some non-traditional hours may be required

MINIMUM QUALIFICATIONS:
- High school diploma or equivalent (GED); or an equivalent combination of education and/or experience which provided the required knowledge, skills, and abilities.
- Working knowledge of Butler County neighborhoods, resources, and social services
- Experience working within a harm reduction setting or working with populations of people who use drug or experiences mental health issues
- Ability to have a flexible schedule and work in a variety of settings while maintaining professional relationship and appropriately managing time
- Understanding of public health prevention and harm reduction concepts with an emphasis on knowledge of treatment and social service programs
LICENSE OR CERTIFICATION REQUIREMENTS:
- Valid Driver’s license in good standing, insurable by the Board of Health’s Policy
