Position Summary:
We are seeking compassionate, experienced, and motivated professionals to join our team to provide Independent Living Skills Training (ILST) services to individuals with Acquired Brain Injuries (ABI). This role requires a deep understanding of brain injury rehabilitation and a strong commitment to helping clients achieve greater independence in their homes and communities.
Key Responsibilities:
- Develop and implement individualized ILST plans based on client goals and abilities
- Provide hands-on training in daily living skills such as meal preparation, budgeting, hygiene, transportation, time management, medication reminders, and community integration
- Monitor client progress and adjust support strategies accordingly
- Document all services provided in accordance with program and state requirements
- Collaborate with interdisciplinary teams, families, guardians, and state agencies
- Promote safety, empowerment, and skill-building for clients in their home and community settings
- Participate in team meetings and ongoing training sessions
Qualifications:
- Minimum of 2 years experience working with individuals with Acquired Brain Injury (ABI)
- Direct experience in providing ILST services is preferred
- Knowledge of person-centered planning and goal setting
- Strong documentation, communication, and organizational skills
- Valid driver’s license and reliable transportation
- High school diploma or GED required
