Description

Butte Home Health is dedicated to providing compassionate and high-quality home health care services. We are seeking a skilled and motivated Home Health Physical Therapist to join our team. The ideal candidate will have experience in delivering physical therapy services in a home setting and a strong commitment to improving the quality of life for our patients.

Key Responsibilities :

Evaluate and Assess : Conduct comprehensive assessments of patients' physical abilities and limitations. Develop individualized treatment plans based on assessment results.
Develop Treatment Plans : Design and implement personalized therapy plans to improve patients' mobility, strength, balance, and overall functional independence.
Deliver Therapy Services : Provide hands-on physical therapy services including exercises, manual therapy, and modalities to help patients achieve their goals.
Patient Education : Educate patients and their families about their conditions, treatment plans, and strategies for maintaining and improving their physical health.
Progress Monitoring : Regularly assess and document patients' progress, adjust treatment plans as needed, and communicate with other healthcare professionals regarding patient care.
Collaboration : Work closely with physicians, nurses, and other healthcare providers to ensure coordinated and effective care.
Compliance : Adhere to all regulatory and safety guidelines, maintaining up-to-date documentation and ensuring that all services are compliant with federal and state regulations.
Maintain Equipment : Ensure that all therapy equipment is maintained and used correctly, and provide recommendations for any necessary equipment modifications.

Qualifications

Qualifications :

Education : Degree in Physical Therapy from an accredited program.

Licensure : Valid state licensure or certification as a Physical Therapist.

Experience : Previous experience in a home health setting preferred but not required. New graduates are encouraged to apply.

Skills : Strong clinical assessment and diagnostic skills, excellent communication and interpersonal skills, and the ability to work independently and manage time effectively.

Physical Requirements : Ability to lift and assist patients as needed, and perform physical tasks associated with therapy services.
