What does a MDS Coordinator do?
As a MDS/Care Plan Coordinator at Butterfield Trail Village, you will play a pivotal role in assessing and monitoring the health and wellbeing of our residents in accordance with the Resident Assessment Instrument (RAI) and relevant legislation. Your responsibilities will include performing comprehensive care area assessments and guiding the care planning process, ensuring that the health and wellbeing of our residents are consistently monitored and accurately documented. Proficiency in MDS 3.0, experience with EMR or EHR systems, and a current LPN or RN license in good standing are required. A minimum of four years in a nursing role within long-term care and two years as an MDS Coordinator is also necessary to ensure compliance with state and federal regulations.
Knowledge and skills required for the position are:
- Current LPN or RN license in good standing with ASBN.
- Minimum of 4 or more years in a nurse role in long term care.
- Minimum of 2 or more years as a MDS Coordinator.
- In depth knowledge of assessment complex coding and regulations.
- Must maintain all certifications and licensure
- as well as any on-going training/education required by OLTC.
- Worked in a EMR or EHR system(s)
- Matrix care experience is a plus.
- Excellent organizational and interpersonal skills including written and oral communication.
- Ability to develop and maintain positive appropriate relationships with residents families, coworkers physicians and their representatives.
- Ability to organize and prioritize job responsibilities and to work independently.
- Must respect confidentiality of resident and coworker information.
- Understanding of Medicare and Medicaid requirement.
- Familiarity with MDS assessment tool, including MDS coding knowledge.
- Understanding of medical terminology, nursing practices and patient care standards.
- Attention to detail to ensure precision in completing assessment and accurate data entry
- Assessment, data collection and documentation skills
- Knowledge of state and federal regulations.
- Proficient in MDS 3.0 ,with an ability to become familiar with BTV-specific programs and software.
