Job Description

We are currently looking for a full time Licensed Mental Health professional. 


Duties include:

  • Treatment Planning
  • Progress Notes Review
  • Assessments
  • Transition/Discharge Summaries

Thorough documentation of screening/assessments, evaluations, notes, contacts made must be accurate and timely.

Position is responsible for coordinating care with the Case Manager and other care providers.

Must be a Licensed Professional Counselor, LPCC, Licensed Social Worker or LISW in Ohio.
