What are the responsibilities and job description for the Mental Performance Coach / Case Manager School Base position at Butterfly Support Services?
Duties and Responsibilities:
- Serve as a liaison, advocate, and coordinator for clients' care, ensuring effective communication among involved parties.
- Provide ongoing education to clients and their families through verbal consultations and written materials.
- Manage a caseload, maintaining accurate records and reports in a timely manner to support client assistance.
- Participate in staff meetings, training sessions, and supervision, contributing as appropriate.
- Meet clients and families at various locations, including homes, schools, libraries, and community facilities.
- Collaborate with school officials, justice system representatives, and healthcare practitioners as necessary.
- Document client interactions promptly, submitting progress notes within 24 hours of meetings.
- Maintain strict confidentiality regarding client information.
- Uphold and adhere to Core Values and operational principles.
- Achieve individual weekly goals consistently.
- Collaborate with Counselors to devise strategies for retaining youth participants.
- Ensure secure storage of Personally Identifiable Information at all times.
- Conduct direct recruitment activities, including public speaking engagements.
- Represent the organization at designated events such as neighborhood activities, job fairs, and resource fairs.
- Develop and nurture connections with referral sources, including colleges, school districts, and youth organizations, to facilitate outreach efforts.
- Interview potential candidates to gather eligibility information and complete applicant documentation.
- Coordinate and communicate with management to align outreach and placement support.
- Complete all necessary documentation for enrollment according to company standards.
- Foster a respectful and safe environment on-site, free from harassment and intimidation, for staff and participants.
- Perform any additional duties as assigned.
Requirements:
- Possession of a valid driver's license with an acceptable driving record and current car insurance.
- Successful completion of a background check.
- Minimum educational requirement of a High School Diploma/GED.
- Age requirement of 21 years or older.
- All credentials will be subject to verification.
Qualifications:
- High School diploma or equivalent is required.
- Preferred experience includes working with youth and/or adults.
