What are the responsibilities and job description for the Home Health Nurse Case Manager position at By the Bay Health?
We are currently seeking a Registered Nurse to serve as a Home Health Nurse Case Manager in Sonoma County and Marin County as needed. The nurse is responsible for providing in-home care for patients, plans and directs services to meet the needs of individuals and families within their home-living environment. The nurse will provide skilled care, education and training to the patient/family based on identified needs to maximize patient rehabilitation and remain safely in the home
By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.
Essential Duties & Responsibilities include (but are not limited to):
- The RN case manager assumes responsibility to coordinate patient care for an assigned caseload.
- Completes an initial comprehensive assessment of patient to determine home care skilled needs.
- Initiates the plan of care and necessary revisions including obtaining written physician orders for changes.
- Provides direct patient care and services in accordance with the plan of care.
Education/Experience:
- BSN degree preferred.
- Minimum of 1 year experience as a Registered Nurse
- Home Health preferred.
Certificates, Licenses, Registrations:
- Current license as a Registered Nurse in the State of California.
- Current license as a Public Health Nurse in the State of California is highly desirable.
- Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.
Pay range:
$60.00 - $74.00 hourly
