The Team/Admissions Nurse provides coverage for Nurse Case Managers to cover patient/staffing needs and processes admissions for eligible hospice patients in coordination with Team Manager.
Schedule:
Monday - Friday; 8:30am-5:00pm

Napa and Solano Counties
By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.
Essential Duties & Responsibilities include (but are not limited to):
- Make in-person nursing visits to patients as assigned, or as indicated when case managing.
- Use the nursing process to assess, plan, intervene and evaluate following patient/family needs and goals.
- Provide effective management of symptoms, including prompt escalation to providers or others as indicated.
- Coordinate patient/family services and prioritization of needs with the members of the transdisciplinary team or outside providers/services.
Education/Experience:
- One year experience as a professional nurse in the past three years or; Bachelor's degree in Nursing from an accredited School of Nursing.
- Experience in home health and/or hospice setting preferred.
Certificates, Licenses, Registrations:
- Current license as a registered professional nurse in the State of California.
- Public Health Nursing certification highly desirable.
- HPNA certification preferred.
- Current California Driver's License and automobile insurance that meets State of California requirements, if driving a car.
Pay Range:
$57.00 - $71.50 hourly
