Position Summary:
The Nurse Family Partnership (NFP) Nurse Home Visitor (NHV) is responsible for providing comprehensive nursing services to pregnant persons and their families. NHV’s are responsible for maintaining the highest standards in clinical and nursing practice with adherence to the NFP national, evidence-based home visitation model, policies, procedures, and guidelines of NFP and CIDS. This position is also responsible for visiting new parents at local hospitals through the Infant Registry Program to enroll eligible and interested families in CIDS programs.
The program goals include improving outcome in the following areas:
- Pregnancy and Birth
- Child Health and Development
- Economic Self-Sufficiency
Qualifications:
Required Education: Bachelor of Science in Nursing (BSN), RN license (in good standing). Preferred Experience: Two-year experience with home visiting, maternal-child health, public health, or mental/behavioral nursing.
Must have excellent written, verbal communication skills and basic computer skills
Must have the ability to lift at least 25 pounds, have an active driver license and vehicle.
Must be able to work flexible hours, including days, evenings and weekends.
Job Duties:
- Works collaboratively with all programs held at CIDS, staff and community partners to ensure continuum of care.
- Provides home visits up to 12 first time pregnant persons/families eligible for NFP in accordance with NFP model.
- Complete all required education for professional development.
- Assess physical, emotional, social, and environmental needs of families as they relate to NFP domains.
- Provides education, support, and referral resources in assisting families in attaining their goals consistent with NFP program and policies.
- Formulates nursing diagnosis based on nursing assessment and client goals.
- Provides information to support appropriate, timely and accurate data into data management systems.
- Cooperate in review and analysis reports for achievements and areas for improvement.
- Maintain incoming referral and intake process.
- Fulfill NHV services consistent with NYS DOH regulations.
- Visit local hospital’s Labor and Delivery floor to enroll new parents in CIDS programs
- Fill out appropriate referral documentation to CIDS programs and partnering agency programs.
- Collaborate with the Infant Registry team to provide CIDS Program Director with monthly reports of the Infant Registry Program.
In this role you will:
NFP provides home visits to first-time, low-income pregnant persons and their families before the mother is at 28 weeks' gestation and continuing until their child turns two years old. In this position, the Nurse Home Visitor will carry a caseload of up to 12 families.
Typical assessments and screenings include blood pressure, infant growth and feeding, parent-child interactions, intimate partner violence, substance use, mental health (PHQ-9 and GAD-7), and infant development with ASQ questionnaires. In response to assessments, nurses utilize interventions such as education, therapeutic communication, motivational interviewing and community referrals.
Job Type: Part Time (16.75-25 hours per week)
Pay: $32,500-$37,000
Schedule: Monday-Friday, Some Evenings, Some Weekends
