What are the responsibilities and job description for the PHC Field Supervisor position at C&L ESPERANZA HOME HEALTH INC?
The primary responsibility for the PHC Field Supervisor is to provide in-home initial training for Primary Home Care (PHC) attendants, conduct Pre-Initiation Activities, conduct monthly supervisory visits for all PHC Consumers and PHC Attendants to assure services are being provided in line with the approved plan of care. They are also responsible for training all PHC Attendants on the call in/out process on initiation of services and continuously as needed. Company car provided.
- High school diploma
- Spanish Bilingual required
- 1-2 years of health care experience
- Reliable transportation
- Skillful in organization of work and in principles of time management.
- Ability to contribute to the quality of care being rendered through constructive communication with primary care staff
- Good communication skills and ability to establish good rapport with other staff members.
- Demonstrates ability to work with patients and employees.
- Must have excellent written communication by writing clearly and informatively, edits work and able to read and interpret legal written information.
- Must practice a high level of confidentiality and be able to effectively respond to questions from employees, managers and outside entities. Must have excellent communication skills, both verbal and written.
- Must have knowledge and proficiency of office computer equipment and software.
Driving & Other Requirements
Must have and maintain a valid state driver license, have a registered and reliable vehicle that meets state law standards, and meets the insurance standards of company. Position requires travel between company sites as well as the community.
