Franciscan is known for our mission of caring. WHAT YOU CAN EXPECT Completes appropriate goal re-assessment and discharge planning.. Performs comprehensive occupational therapy evaluations, treatment and related follow-up services including discharge planning. Documents and submit patient reports and charges within the established timeframe. Collaborates with patient and other healthcare personnel or outside agencies regarding coordination of care. Develops a treatment plan with measurable goals based on patient assessment. Provides occupational therapy, including selection and use of assistive equipment. Trains and educate patients and family members or other support and document understanding of treatment plan and goals.
