Collaborates with the inter-disciplinary treatment team. Documents patients seen. Essential Functions: Plans physical therapy program for each patient based on functional, timed, measurable and objective goals while safely using modalities and appropriate exercise or other interventions as indicated. Applies diagnostic and prognostic muscle, nerve, joint and functional ability tests. Directs and aids patients in active and passive exercises, muscle re-education and functional training using pulleys, weights, steps, inclined surfaces, assistive devices and other functional aide equipment devices. Ability to function in a healthcare team and communicate effectively in a positive, professional manner with patients, physicians and various interdisciplinary team members. Provides written instructions and therapeutic procedures to be continued by the patient and/or caregiver. Ability to utilize electronic medical record documentation/charge systems to effectively evaluate, record, and report on patient progress for review by other members of the rehabilitation team. Performs other miscellaneous and related duties as assigned. Additional Job Description Must be licensed in the State of South Carolina and possess a bachelor’s degree in physical therapy. Current CPR Certification. Some experience is preferred but not required. DPT preferred but not required.
