Job Description Summary Job Summary Provides comprehensive and continuing patient care through the provision of social work care coordination to patients and their families. Functions as an integral member of a collaborative, interdisciplinary team, using assessment, planning, implementing, coordinating, monitoring, and evaluation skills. II. Minimum Qualifications A Degree in Social Work.LMSW, LISW, or LBSW license with the State of South Carolina required within 6 months from date of hire. One year of related social work experience is required preferably in a healthcare setting. III. Work Environment Primarily works in the Behavioral Health unit. Must be able to handle emergency or crisis situations up to 70% of the time and demonstrate emotional stability. Subject to long and irregular hours. Occasional standing and walking. Must be able to handle varying or unpredictable situations. IV. Supervisory Responsibilities None. V. Financial Responsibilities None. VI. Essential Functions 1. Interviews patients and family members to obtain information about home environment, family relationships, health history and patient personality traits. Assesses data gathered in terms of the medical plan of treatment, available social service programs and plans and implements discharge arrangements. 2. Educates patients and families in understanding, accepting, and following medical recommendations. 3. Initiates screening of discharge planning needs on high risk patients. Coordinates discharge planning activities with input from the appropriate inter-disciplinary team. Stays abreast of available community resources and utilizes resources to help patients in smoothly moving to the next appropriate level of care. 4. Interprets the nature of the illness and prescribe course of treatment to patient and family. Communicates family issues to other staff members one to one and in patient care conferences. 5. Coordinates with the inter-disciplinary team members and community resource personnel. 6. Adheres to the department policy for documentation and provides monthly statistics as required. 7. Provides crisis and situation-oriented counseling to patients/families. 8. Performs other miscellaneous and related duties as required. VII. Physical Requirements Physical requirements are primarily related to the essential functions of any job. In order to perform the essential functions of this position, you must be able to stand, walk sit, use hands, stoop, talk, and hear. Must be able to lift or exert energy up to 10 pounds 100% of the time. Must have close vision (clear vision at 20 inches or less), and distance vision (clear vision at 20 feet or more). VIII. Mental Requirements Must possess the ability to read and interpret business or safety manuals, clinical or technical procedures or regulations. Ability to write reports, correspondence and/or manuals. Must have the ability to effectively present information and respond to questions from groups, patients, families, other employees, and medical staff. Must have the ability to perform more complex math functions to include calculating percentages, averages, volume, ratio and interpreting graphs-using numbers, money and units of measurement. Must have the ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to deal with concrete and abstract concepts and interpret verbal, nonverbal and written instructions. IX. Population Served This position primarily interacts with children, adolescents, adults and aging adults. All staff members must be able to demonstrate the knowledge and skill necessary to meet the physical, psycho-social, educational, safety, and related needs of the persons in the assigned work area. This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions. Additional Job Description Education: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
