Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care. Consistently evaluate the effectiveness of care provided to the patient. Perform interventions to meet holistic needs of patient, under the direction of the Medical Director, and under the leadership of the Client Resource Manager.
